Some practitioners and coaches may choose to send clients intake forms before starting work together. The purpose of an intake form is to gather essential information about a client's background, mental health history and current concerns, providing therapists and coaches with valuable insights to tailor their approach and address the client's specific needs effectively.

If this is a part of your practice, as with any form, it is important to be clear with the client why you are asking for this information and how it can be helpful to you/the work.

In some countries, or in certain US states, completing an intake form might be a mandatory requirement under local laws or regulations. If this applies to your practice, please ensure that it is explicitly communicated to your clients that the form must be completed prior to the session, and that the session will not go ahead without it.

All forms collected with data and personal information should be treated as confidential and stored appropriately in line with local protection laws and guidance from your accrediting bodies. See here for more information: Storing session notes

We ask that your forms do not contain the clinical outcome questionnaires e.g. PHQ-9 and GAD-7 as these are embedded into the online system and it may frustrate clients to have to repeat the questionnaires several times.
